Job ResponsibilitiesIn this role, you will:
- Conduct thorough assessments of patients' physical and emotional needs
- Develop and implement individualized care plans to address complex health issues
- Communicate effectively with patients, families, and the care team to ensure seamless transitions and optimal outcomes
To succeed in this role, you must possess:
- A degree in nursing or a related field from an accredited institution
- Current RN licensure in the applicable state(s)
- Basic Life Support (BLS) certification
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
