Responsibilities- Monitor and maintain resident charts and clinical progress notes.
- Communicate with physicians, staff, and family members regarding changes in resident needs or conditions.
- Facilitate coordination of resident services with other service providers.
- Oversee medication administration procedures and discuss any necessary corrections or errors with staff.
- Provide alternating supervisory visits in coordination with the RN and supervise medication practices and medical equipment.
- Ensure staff clearly understand and follow all resident care procedures.
- Maintain a current state license as an LPN.
- Minimum of one year's experience in home health, assisted living, or long-term care.
- Good organizational skills with the ability to self-start and follow through with initiatives.
- Exhibits a caring heart with a passion to care for the elderly.
The LPN position typically works Monday to Friday, 9:00am to 5:00pm, and is required to work on call every third weekend.
