Job SummaryThe Case Manager will provide Ryan White and DRS Home Services Case Management, promoting empowerment and self-sufficiency among clients. This includes conducting initial assessments, developing individualized service plans, coordinating services, and monitoring client progress. The ideal candidate will have excellent communication and interpersonal skills, with the ability to build strong relationships with clients and community partners.
Key Responsibilities- Build strong relationships with clients, providing emotional support and guidance to help them achieve their goals.
- Conduct initial assessments and develop comprehensive service plans, ensuring clients receive the necessary resources and support.
- Coordinate services, including referrals to hospitals, physicians, and community resources, to ensure clients receive timely and effective care.
- Monitor client progress, making adjustments to service plans as needed to ensure optimal outcomes.
- Provide advocacy and support to clients, helping them navigate the healthcare system and access necessary resources.
- Bachelor's or Master's degree in Social Work or a related field.
- Experience working with clients living with HIV/AIDS, preferably in a case management or social work role.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and community partners.
- Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and deadlines.
- Strong attention to detail and ability to maintain accurate records and reports.
