About the Role:

We are seeking a highly skilled and compassionate Patient Care Manager to join our hospice team at SouthernCare. This is a full-time position that requires a Registered Nurse with a minimum of Associate's Degree in Nursing and three years of direct patient care experience in a clinical setting.

The ideal candidate will have a strong understanding of hospice care and be able to lead a team in providing high-quality patient care. The Patient Care Manager will be responsible for the day-to-day operations of the hospice site, including managing clinical and administrative functions, responding to phone referrals, and participating in on-call nursing schedules.

Key Responsibilities:

  • Manage the site in the absence of the Executive Director or Administrator
  • Oversee clinical and administrative functions
  • Respond to phone referrals in a timely and professional manner
  • Participate in on-call nursing schedules and provide direct skilled bedside nursing care

About You:

We are looking for a candidate with a strong educational background, including a Registered Nurse with a minimum of Associate's Degree in Nursing and a Bachelor's Degree in Nursing preferred. The ideal candidate will have a minimum of three years of direct patient care experience in a clinical setting and a minimum of one year of management/supervisory experience. Additionally, the candidate should have a strong understanding of hospice care and be able to lead a team in providing high-quality patient care.
