We are seeking a highly skilled Rehabilitation Social Worker III to join our team at AdventHealth Winter Park. As a key member of our case management team, you will play a vital role in coordinating social work and case management services to patients and their families.
Key Responsibilities- Coordinate social work and case management services to patients and their immediate families.
- Serve as the client advocate and assist in the coordination of the rehabilitation process from admission to discharge.
- Adhere to AdventHealth Corporate Compliance Plan and to all rules and regulations of all applicable local, state and federal agencies and accrediting bodies.
- Provide exceptional customer service and maintain relationships that are respectful to all.
- RN or Bachelor's degree in Rehabilitation or Social Services.
- Basic Life Support (BLS) certification.
- MSW.
- Experience in Rehabilitation.
