Job Title: Senior Care Manager - LPN
About the RoleWe are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Senior Care Manager. As an LPN, you will play a vital role in providing high-quality care and services to our residents, ensuring their physical, emotional, and social well-being.
Responsibilities:- Administer medications and treatments safely and effectively, following established protocols and guidelines.
- Conduct service and health updates, collaborating with the Resident Care Director and other healthcare professionals to develop and implement individualized care plans.
- Participate in daily cross-over meetings and contribute to team decision-making to ensure seamless care delivery.
- Maintain accurate records and documentation, adhering to regulatory requirements and Sunrise Senior Living policies.
- Engage with residents, families, and colleagues to promote a positive and supportive community environment.
- Current LPN license or equivalent; certification in CPR and First Aid is also required.
- Minimum 1 year of experience working with seniors, preferably in a long-term care setting.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and collaboratively as part of a multidisciplinary team.
- Competitive salary: $65,000 - $80,000 per annum, depending on experience and qualifications.
