Job SummaryWe are seeking a skilled RN Care Manager to join our team. As a RN Care Manager, you will be responsible for planning and prioritizing care for individuals and populations of patients, focusing on strategies that promote optimal health within populations.
Key Responsibilities:- Assess internal and external referrals to identify patient/significant others' needs, level of intensity, insurance benefits, and other patient resources.
- Develop a plan of care and make recommendations to PCPs, specialists, and other members of the healthcare team regarding care management strategies.
- Assess the educational needs of patients, families, and members of the healthcare team and develop and implement appropriate teaching strategies or make appropriate referrals.
- Communicate and collaborate with patient/significant others/providers/payers to coordinate services that improve access to appropriate services across the continuum of care.
- Measure patient care outcomes, interpret reports, and analyze data trends for groups of patients.
- Document patient data, plan, interventions, and outcomes according to department guidelines.

To be successful in this role, you will need:
- A Bachelor's Degree in Nursing.
- A Registered Nurse license in the State of Michigan.
- 5 years of relevant clinical experience, preferably in a managed care environment.
- 2 years of relevant experience in clinical nursing and/or case management/managed care or a related field.
