Care Coordinator

Join Sunflower Health Plan in transforming the health of our communities, one person at a time. As a Care Navigator, you will play a vital role in developing and coordinating care management activities to provide quality, cost-effective healthcare outcomes for our members.
Key Responsibilities:- Evaluate member needs, barriers to care, and resources available to recommend and facilitate the best outcome.
- Develop personalized care plans and work with providers, specialists, and community resources to ensure timely access to care.
- Provide psychosocial and resource support to members and caregivers, and collaborate with healthcare providers to address identified needs.
- Monitor progress towards care plan goals and collaborate with healthcare providers for plan revision or address identified member needs.
- Collect, document, and maintain member information and care management activities to ensure compliance with state, federal, and third-party payer regulators.
- Bachelor's degree and 2-4 years of related experience.
- Current state clinical license preferred.
- Competitive pay: $37.25 per hour.
