Key Responsibilities

  • Provide direct patient care services, including assessing patient needs, developing and implementing care plans, and coordinating with healthcare teams.
  • Act as a patient advocate, ensuring that patients' rights and preferences are respected and upheld.
  • Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and spiritual care professionals, to deliver holistic care.
  • Educate patients and their families on care management, providing guidance and support to promote independence and well-being.
  • Manage patient clinical records, maintaining accurate and up-to-date documentation.

About You

Requirements:

  • Current RN license in the state of employment (or eligibility to obtain)
  • At least 1 year of experience in hospice, home health, ICU, geriatric, or oncology nursing (strongly preferred)
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations
  • Comfortable providing care in a variety of settings, including patients' homes, hospitals, and long-term care facilities
  • Compassionate and empathetic approach to patient care, with a commitment to delivering high-quality, patient-centered services
