Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at Group Main Stream. As a Case Manager, you will play a vital role in supporting individuals with complex needs, promoting their empowerment, and ensuring their health and safety.

Key Responsibilities
  • Develop and maintain relationships with individuals, their families, and other agencies to identify and address their needs.
  • Collaborate with providers to ensure seamless service delivery and promote person-centered planning.
  • Conduct regular assessments and evaluations to inform service planning and ensure compliance with regulations.
  • Provide advocacy and support to individuals, promoting their rights and interests.
  • Ensure accurate and timely documentation of services and outcomes.
  • Participate in quality improvement initiatives and contribute to the development of best practices.
Requirements
  • Bachelor's degree in a relevant field, such as psychology, social work, or human services.
  • At least one year of experience in case management or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid Maine state driver's license.
