Job Title:

Embedded Care Manager

About the Role:

Vaya Health is seeking a skilled Embedded Care Manager to join our team. In this role, you will provide proactive intervention and coordination of care to eligible Vaya Health members and recipients. You will work closely with members, care teams, and community stakeholders to ensure that individuals receive appropriate assessment and services.

Key Responsibilities:

  • Conduct comprehensive bio-psycho-social assessments addressing social determinants of health, mental health history and needs, physical health history and needs, activities of daily living, access to resources, and other areas to ensure a whole person approach to care.
  • Develop person-centered care plans to help define what is important to members for their health and prioritize goals that help them live the life they want in the community of their choice.
  • Work with members to identify barriers and help resolve dissatisfaction with services or community-based interventions.
  • Provide crisis intervention, coordination, and care management if needed while with members in the community.
  • Support Transitional Care Management responsibilities for members transitioning between levels of care.
  • Coordinate Diversion efforts for members at risk of requiring care in an institutional setting.

Requirements:

  • Bachelor's degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area.
  • Two years of experience working directly with individuals with behavioral health conditions.
  • North Carolina Qualified Professional status with the population served.
