Key Responsibilities:
- Assess physician medication orders for appropriateness regarding drug selection, dosage, potential interactions, and administration routes. Provide recommendations for therapy modifications to physicians and facility staff as necessary.
- Maintain a comprehensive patient/resident database that includes current and historical medical information, relevant clinical and laboratory data, and concurrent medications (including over-the-counter and home remedies).
- Ensure that necessary laboratory tests for monitoring high-risk conditions are ordered, assessed, and discussed with the healthcare team.
- Collaborate with the healthcare team to deliver clinical and technical support as required.
- Document all communications with physicians, nurses, other healthcare team members, and patients/residents and their families in the medical record.
- Implement policies and procedures for the safe procurement, storage, distribution, and usage of medications and biologicals, as well as their disposal.
- Obtain and maintain all required documentation in compliance with federal and state regulations and company policies.
- Notify the Pharmacy Manager of any medication-related errors.
- Assist facility staff with procedures related to the safe and effective management of medications.
- Perform additional tasks as assigned.

Qualifications:
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- Minimum of one year of experience in long-term care.

Licenses/Certifications:
- As mandated by government regulations and business requirements.
- Detail-oriented, goal-driven, team-oriented, with strong oral communication skills.
