Job Title: Physical Therapist - Home Health

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Salisbury, Massachusetts. As a Physical Therapist, you will play a vital role in providing high-quality care to our clients in the comfort of their own homes.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to work with our senior living office in Seabrook, Hampton, and Exeter, NH. As a Physical Therapist, you will be responsible for performing home health visits, developing and implementing individualized care plans, and educating clients and their families on rehabilitative care and activities necessary for independent living.

Responsibilities:
  • Conduct home visits to assess clients' physical needs and develop personalized care plans.
  • Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
  • Develop and implement manual therapeutic exercises, gait training, and other interventions to improve clients' physical function.
  • Continuously assess and revise care plans to ensure optimal outcomes.
  • Educate clients, family members, or caregivers on rehabilitative care and activities necessary for independent living.
Requirements:
  • A current New Hampshire Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
