Job Summary:We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Home Health Physical Therapist, you will work closely with our multidisciplinary team to develop and implement individualized care plans that promote our clients' health, safety, and independence.
Responsibilities:- Conduct home visits to assess clients' physical needs and develop personalized care plans.
- Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
- Develop and implement manual therapeutic exercises, gait training, and other interventions to improve clients' physical function.
- Educate clients and their families on rehabilitative care and activities to promote independent living.
- Accurately document observations, interventions, and evaluations in our state-of-the-art electronic medical records system.
- A current New Hampshire Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
