Job Summary:

The PrEP Coordinator will assist the Program Coordinator with program implementation, helping participants secure PrEP, medication adherence, and retention on PrEP. This role will also involve delivering HIV and HCV test results, conducting follow-ups, and participating in PrEP promotional campaigns.

Key Responsibilities:
  • Provide confidential assessments to determine HIV prevention needs of participants
  • Develop PrEP/PEP resources and network with providers
  • Ensure access to biomedical prevention services, including PrEP/PEP and supportive services
  • Provide access to STI screening and collaborate with project staff on advertising and media promotion
  • Responsible for HIV antibody testing on-site and through outreach
  • Offer Partner Services to individuals who test positive for HIV
  • Perform follow-up risk assessments and collaborate with DIS for linkage and re-linkage to prevention services
  • Maintain communication between inter and intra-agency services providers
  • Document program activities and report outcomes to the CDC
Requirements:
  • Minimum of two years of experience in patient navigation, case management, education, or human services
  • Experience in HIV/AIDS services and knowledge of HIPAA
  • Excellent working knowledge of the New Haven community and social and health service agencies
  • Specialized training or experience in HIV prevention interventions, PrEP/PEP, and STIs
  • Ability to work in a clinical setting with providers and medical personnel
Preferred Qualifications:
  • Bilingual - English/Spanish
  • Knowledge of computer applications and phone etiquette
  • Ability to multitask, work well in teams, and be highly detail-oriented
Working Conditions and Physical Requirements:
