Job Summary:
We are seeking a highly motivated Community Support Specialist - Mental Health Advocate to join our team. As a key member of our Hospital Transition Team, you will play a vital role in providing short-term services designed to help individuals maintain psychiatric progress and connect with essential services and benefits after hospital discharge.
Responsibilities:
- Implement individual habilitation plans to instruct consumers in daily living skills, socialization, leisure, and recreational activities.
- Document services provided in progress notes, ensuring timely completion and adherence to company policies.
- Supervise and assist consumers with general daily living skills, such as meal preparation, grooming, bathing, eating, money management, and transportation to community services/activities when required.
- Encourage consumers to make decisions independently, promoting maximum independent living skills and self-sufficiency.
- Monitor progress notes, charts, programming materials, and maintain all required records.
- Submit documentation by specified deadlines and report any changes in consumer condition, family situation, or needs to relevant personnel.
- Adhere to HIPAA regulations, company policies, and procedures, including emergency services and medication administration guidelines.
Requirements:
- A high school diploma or GED equivalency
- North Carolina state certification as a Peer Support Specialist (PSS), with a minimum of 40 hours of approved training
- A valid NC Driver's License and at least 8 years of driving experience
- No substantiated findings on the NC Personnel Registry/Health Care Program or the HHS/OIG List of Excluded Individual/Entities
