About the Role
We are looking for a motivated and compassionate Physical Therapist to provide high-quality patient care in the comfort of clients' homes. As a Home Health Therapist, you will be responsible for assessing patients' needs, developing and implementing individualized care plans, and providing therapy services to promote recovery and independence.
Responsibilities
- Conduct thorough assessments of patients' physical and functional abilities.
- Develop and implement individualized care plans that address patients' unique needs and goals.
- Provide physical therapy services, including manual therapeutic exercises, gait training, balance, and other interventions, to promote patients' recovery and independence.
- Collaborate with other healthcare professionals to ensure comprehensive care and support.
- Document patient progress and outcomes accurately and thoroughly.
Requirements
- A current Maryland Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
