Position Overview:

Key Responsibilities:

  • Report directly to the Administrator, Executive Director, or Patient Care Manager Senior, maintaining primary oversight and professional management of each patient.
  • Serve as the primary communication link between the physician, patient/family, and hospice team.
  • Deliver direct patient care services to hospice patients while providing supportive care to their families, managing a reasonable caseload, and fostering trusting relationships.
  • Empower caregivers by educating them on how to care for their loved ones.
  • Collaborate with an interdisciplinary team, including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff, to ensure comfort, peace, and dignity for patients.
  • Assess patient and family needs, assisting in the development and implementation of individualized care plans.
  • Maintain accurate clinical records for all assigned patients.
  • Participate in quality assessment and performance improvement initiatives.

Candidate Profile:

  • A compassionate heart, demonstrating kindness and empathy towards patients and their families.
  • Comfortable providing care in various home settings.
  • Ability to remain flexible and composed while supporting others during crises.
  • Valid RN license in the state of employment (or eligibility to obtain).
  • A minimum of 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology settings is strongly preferred.
  • Additional preferred experience includes roles such as hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
