As a Hospice RN/ RN Case Manager, you will have the opportunity to provide direct patient care services and supportive care to the patient's family, while empowering patient's caregivers through education and collaborating with medical staff to provide comfort, peace, and dignity.
Responsibilities:
Directly report to the Administrator, Executive Director or Patient Care Manager Senior
Act as the primary liaison between physician, patient/family, and hospice team
Provide direct patient care services and supportive care to the patient's family
Empower patient's caregivers through education
Manage and collaborate with medical staff to provide comfort, peace, and dignity
Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient
Participate in company quality assessment and performance improvement programs
Requirements:
Hospice Heart:
Compassionate, kind, and empathetic
Comfortable providing care wherever the patient calls home
Current RN license in the state of requested employment
1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
Current driver's license and automobile insurance
Current CPR certification
