Nurse Care Specialist

As a Weekend On-Call Registered Nurse with Caris HealthCare, an affiliate of NHC, you will play a vital role in providing exceptional patient care and support to our patients and their families.
Responsibilities:
Provide nursing care to patients in their homes, focusing on their physical, emotional, and social needs.
Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
Conduct thorough assessments of patients' conditions, identifying areas for improvement and developing strategies to address them.
Educate patients and their families on disease management, medication adherence, and healthy lifestyle choices.
Requirements:
Must be a Registered Nurse licensed in the state of operation.
Nursing experience required, with experience in Hospice or Home Health preferred.
Possess a sound knowledge of nursing principles, procedures, and elements of patient family teaching.
