The Social Services Assistant plays a vital role in supporting social service personnel by delivering administrative assistance throughout the patient journey, from admission to discharge.

Position Highlights:
  • Engage with patients and their families to provide essential support and resource information.
  • Address concerns raised by patients and their families.
  • Facilitate smooth patient transfers within the facility to ensure effective transitions and adjustments, documenting as necessary.
  • Participate in healthcare decision-making processes under the guidance of qualified social workers or consultants.

Qualifications:
  • Associate degree in human services with at least two years of experience in long-term care is required.
  • Must hold any state-required certifications.
