Key Responsibilities:

  • Provide direct assistance with activities of personal care, including bathing, dressing, and toileting, under the supervision of a Registered Nurse or Therapist.
  • Support patient care by reviewing care plans and notifying Case Managers of any issues or concerns.
  • Assist with daily living tasks, such as meal preparation, house cleaning, and laundry, as outlined in the patient's care plan.
  • Communicate effectively with patients, family members, and healthcare providers to ensure seamless care coordination.
  • Complete mobile care reports and mileage logs accurately and in a timely manner.

Requirements:

  • Certified Nursing Assistant (CNA) certification
  • Experience working in home health care or hospice settings
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
