Key Responsibilities:
The Hospice Aide is responsible for delivering personal care and related services in the home for individuals who are ill or disabled. This includes assisting patients with at least two activities of daily living (ADLs). As a Hospice Aide, your duties will include:

  • Providing assistance with at least two of the following ADLs, including personal hygiene
  • Supporting patients with medication management and prescribed exercises
  • Performing incidental household tasks essential to patient health
  • Adhering to the Plan of Care (Instruction Sheet) as directed and making necessary updates during scheduled visits
  • Ensuring the application of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) to provide Age-Friendly Care

Qualifications:

  • Certification as a Nurse Aide (CNA) is required
  • Must possess a valid driver's license, maintain a good driving record, and have access to a reliable vehicle
  • High school diploma or GED is preferred
  • Ability to work effectively within a multi-disciplinary team
  • Capability to perform in field situations
