Ambulatory Care Nurse

Key Responsibilities:- Conduct phone triage and patient education
- Schedule appointments and referrals
- Coordinate patient care and intake
- Perform other related tasks as needed
- Graduate of an accredited school of nursing
- Valid RN license in the state of Indiana
- One year or more of nursing experience
- Geriatric experience preferred
- Excellent communication and team skills
