Job Title: Ambulatory Registered Nurse

At Community Health Network, we are seeking a skilled Ambulatory Registered Nurse to join our Healthy Aging Transition Services (HATS) care team. As a key member of our team, you will be responsible for conducting phone triage, patient education, scheduling appointments, referrals, prescription refill, care coordination, patient intake, and other related tasks.

About the Role:
  • Graduate of a National League for Nursing (NLN) accredited school of nursing or three years of related professional nursing experience.
  • Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana.
  • One year or more of nursing experience required.
  • Geriatric experience preferred.
  • Previous medical office/outpatient experience preferred.
  • Phone triage experience a plus.
