Job Title: LPN: Licensed Practical Nurse

Job Summary:

We are seeking a skilled LPN to provide direct care and treatment to individuals in our assigned program(s). The ideal candidate will have a minimum of one year of acute or long-term care experience and be able to lift a minimum of 50 pounds.

Responsibilities:
  • Provide direct care and treatment to individuals in assigned program(s) through a variety of nursing procedures and medication administration.
  • Respond and assist in medical emergencies and take appropriate action with guidance from RN.
  • Observe and monitor individuals with specific health problems and those at risk and report pertinent findings to RN.
  • Document all current health-related information in nurses' notes and/or medical record.
  • Check nursing equipment as indicated.
  • Assist RN in annual assessment of consumers in day programs.
  • Keep medical files current and complete.
  • Attend nursing/program meetings as scheduled.
  • Communicate with other service providers, program staff, administration, etc.
