Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at Northern Rivers Family of Services. As a Nurse-LPN, you will play a vital role in providing medical care and services to a youth and adolescent population with emotional, behavioral, and physical needs.

Responsibilities
  • Complete nursing observations and report findings to the registered nurse
  • Review lab and test results, reporting critical values to RN and providers
  • Manage medications, including ordering and obtaining prior authorizations
  • Dispense medications and communicate with medical director/providers regarding client health/behavior
  • Provide client and family education regarding medications and treatments
  • Collaborate with staff regarding concerns and behaviors
  • Participate in Interdisciplinary Medication Review meetings
  • Nursing documentation using EHR
  • Update discharge planning needs
  • Perform other duties as assigned within scope of practice and based on agency needs
Requirements and Education
  • LPN Licensed in New York State
  • 1 to 2 years of experience
  • Experience working with youth and adolescent population desired
