As a HCBS Wait List Care Coordinator, you'll play a critical role in ensuring that individuals receive services in a supportive, effective, efficient, timely, and cost-effective manner.

Key Responsibilities
  • Assess, plan, and implement care strategies that are individualized by the individual and directed toward the most appropriate, least restrictive level of care.
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
  • Ensure the care plan/plan of service includes covered services, EPSDT, in lieu of services, value-added benefit, informal supports, and addresses social determinants of health.
  • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all persons.
  • Advocate for persons and families as needed to ensure the persons' needs and choices are fully represented and supported by the health care team.
  • Conduct home visits in coordination with the person and care team, which may include a community service coordinator.
  • Conduct in-person visits, which may include nursing homes, assisted living, hospital, or home.
  • Serve as a resource for community care coordinator, if applicable.

Requirements

To be successful in this role, you'll need:

  • Bachelor's Degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or related human services area.
  • 1+ years of experience working with persons with long-term care needs.
  • 1+ years of experience working within the community health setting in a healthcare role.
  • Intermediate level of computer proficiency, including the use of Microsoft Outlook, Teams, and multiple web applications.
  • Ability to travel 75% of the time for the role.
  • Valid driver's license, current automobile insurance, and access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area.
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information and live in a location that can receive a UnitedHealth Group-approved high-speed internet connection or leverage an existing high-speed internet service.
Preferred Qualifications

We're looking for candidates with:

  • Licensed Social Worker or clinical degree.
  • 1+ years of experience working with persons receiving services on one of the home and community-based waivers in KS.
  • Background in managing populations with complex medical or behavioral needs.
  • Experience with electronic charting.
  • Experience with arranging community resources.
