Nursing Admissions Specialist

The Nursing Admissions Specialist plays a crucial role in a home care environment, facilitating and coordinating hospice services for patients in collaboration with the Interdisciplinary team. This position involves conducting initial nursing evaluations and making treatment choices that align with the preferences and needs of both the patient and their family. Additionally, the Admissions Nurse is responsible for collecting essential contact details, reviewing and documenting the patient's medication regimen, and obtaining a comprehensive medical history.
Work Schedule: Full-time with five 8-hour shifts per week, including one weekend per month and two holidays annually. Weekend-only shifts may also be available if preferred.
Health Requirement: Completion of the COVID-19 vaccination is mandatory upon commencement of employment.
Key ResponsibilitiesThe Nursing Admissions Specialist will adhere to public health nursing standards, practices, and policies while providing and coordinating hospice care for patients and their families. This role includes overseeing LPNs, CNAs, Unit Secretaries, and Volunteers, and may require rotation to various regions or departments as needed.
QualificationsExperience: A minimum of one year of experience as a staff nurse in a similar setting is required. Experience in hospice and/or palliative care is highly preferred.
Education: A degree in Nursing from an accredited institution is necessary, with a preference for candidates holding a BSN degree.
Licensure: Candidates must possess a valid nursing license from the appropriate governing body for practice in the relevant jurisdiction. Licensure from a compact state is acceptable. Certification in Healthcare Provider BLS (CPR) is required for practice in the relevant states. A CHPN certification is an added advantage.
Driver's License: A valid driver's license in good standing and access to a vehicle during work hours is essential.
