Chapters Health System is seeking a highly skilled and compassionate Admissions Registered Nurse to join our Hospice Care team. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional patient care and support to individuals and their families.
Key Responsibilities- Conduct initial nursing assessments and develop individualized care plans in collaboration with the interdisciplinary team.
- Provide and arrange for Hospice care services, ensuring seamless transitions and continuity of care.
- Supervise and mentor LPNs, CNAs, Unit Secretaries, and Volunteers to ensure high-quality patient care and support.
- Collaborate with healthcare providers, payers, and community resources to ensure comprehensive care coordination.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Minimum of two years of public health/community health, med surg, or related nursing experience.
- Experience in home care, hospice, and/or oncology nursing preferred.
- Diploma or Associates degree in Nursing from an accredited nursing program; BSN preferred.
- Licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice.
- Healthcare Provider BLS (CPR) certification required.
- BSN degree in Nursing.
- Experience working with diverse patient populations and communities.
- Strong communication and interpersonal skills, with ability to work effectively in a team environment.
- Ability to adapt to changing priorities and deadlines, with a focus on delivering high-quality patient care.
