Job Summary

Chapters Health System is seeking a highly skilled Registered Nurse to join our team as an Admissions Specialist. In this role, you will be responsible for providing and arranging for Hospice care for patients and families in a collaborative relationship with other members of the interdisciplinary team.

Key Responsibilities
  • Conduct initial nursing assessments, judgments, and treatment decisions based on patient and family needs and wishes in accordance with professional standards.
  • Supervise LPNs, CNAs, Unit Secretaries, and Volunteers to ensure high-quality patient care.
  • Develop and implement care plans that meet the unique needs of each patient and family.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Provide education and support to patients and families to promote self-care and independence.
Requirements
  • Minimum of two years of public health/community health, med surg, or related nursing experience.
  • Experience in home care, hospice, and/or oncology nursing preferred.
  • Diploma or Associates degree in Nursing from an accredited nursing program. BSN preferred.
  • Licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice.
  • Healthcare Provider BLS (CPR) certification required.
Preferred Qualifications
  • BSN degree.
  • Experience working in a fast-paced environment with multiple priorities.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
