The Pediatric Care Provider will be tasked with evaluating, planning, and initiating the necessary care for both Adult and Pediatric populations within the diverse communities served by Permanent Placement Resources, LLC. The provider will adhere to Board-approved evidence-based protocols to ensure optimal patient outcomes.

- Possesses academic and clinical training suitable for the Provider role.

Must have graduated from an accredited college or university, completed an accredited residency program, and hold a valid medical license in the state of Mississippi. Additional requirements as defined by the Chief Medical Officer must also be met. The individual should be self-motivated, directed, and demonstrate the ability to work both independently and collaboratively within a team.

- Collaborates with Advanced Practice Providers as directed by the CMO, including monthly chart audits and quarterly face-to-face visits as required.
- Initiates the implementation of care plans involving patients, families, and relevant team members.
- Leads the Patient-Centered Medical Home care team for an assigned patient panel.
- Ensures follow-up plans are established for ongoing monitoring as needed.
- Maintains accurate electronic medical records with timely entries.
- Safeguards the privacy and confidentiality of patient information.
- Prepares reports or follow-up requests from colleagues and patients promptly.
- Assists in developing and implementing clinical protocols based on evidence-based practices when requested by the Chief Medical Officer.
- Conducts physical examinations and appropriate procedures safely and according to established protocols.
- Effectively communicates updated medical information related to clinical outcomes to relevant staff members.
- Contributes as a team member to achieve quality clinical outcomes.
