Job Summary:

The Post-Hospital Transition Navigator will play a pivotal role in ensuring the seamless transition of patients to post-acute environments. This position will collaborate with healthcare providers, care managers, and social work teams on transitions of care.

Key Responsibilities:

  • Conduct discharge phone calls to ensure a smooth transition of care
  • Communicate test results to patients and their families
  • Facilitate patient-provider communication to address any concerns or questions
  • Coordinate follow-up appointments as needed

Requirements:

  • Bachelor of Science in Nursing (BSN) degree required; Associate's degree with significant relevant experience considered
  • Current Massachusetts RN licensure
  • Three years or more of medical-surgical inpatient nursing care experience
  • Three years or more of inpatient or outpatient care coordination/case management/discharge planning experience
  • Bilingual (English and Portuguese or Spanish) required; trilingual (English, Spanish, Portuguese) preferred
  • Demonstrated knowledge of coordinating care across various healthcare settings
  • Strong working knowledge of Microsoft Office products
  • Working knowledge of database management and reporting
  • Solid problem-solving skills
  • Detail-oriented
  • Ability to manage complex information
  • Familiarity with insurance prior authorization processes
