Job Summary

We are seeking a highly skilled Case Manager to join our team at the Milford Fire Department - Massachusetts. The successful candidate will be responsible for coordinating and facilitating the management of care for ED patients, both inpatient and observation.

Key Responsibilities
  • Evaluate incoming patients and direct the appropriate discharge disposition (Home, Post-acute Services, Inpatient or Observation) from the ED.
  • Collaborate with the admitting teams to ensure the correct initial admission status (INPT or OBS) at the time of admission using Interqual criteria.
  • Communicate effectively with healthcare team members and family/care givers to ensure seamless patient care.
  • Make sound judgments with knowledge of current medical care technology and ancillary supports.
Requirements
  • Bachelor of Science in Nursing required.
  • Current Massachusetts licensure required. CCM preferred.
  • 3-5 years of hospital-based case management with emergency department experience preferred.
  • Minimum of three years of medical, surgical, ICU or ED nursing experience required.
