The RN Care Manager at Galveston County Health District is responsible for overseeing the care of high-risk, chronic illness patients to promote effective education, self-management support, and timely access to healthcare for optimal quality and financial outcomes.

Key Responsibilities:

  • Collaborate with healthcare providers to identify patients for care management
  • Conduct holistic assessments and formulate care management plans
  • Evaluate and revise care plans as needed
  • Utilize community resources for patient care
  • Promote patient self-management and wellness
  • Maintain accurate documentation
  • Respond to public health emergencies as needed

Requirements:

  • Bachelor's in nursing
  • Registered Nurse license in Texas
  • Minimum 2 years of case management experience
  • Experience with Chronic Care and Transitional Care Management preferred
  • Strong clinical knowledge of chronic conditions
  • Excellent communication and problem-solving skills
  • Proficient in MS Office and electronic health record software
