Job Summary:

Kaiser Permanente is seeking a highly skilled Social Medicine Specialist to join our team at Moanalua Medical Center. As an integral member of our emergency department care team, you will provide psychosocial assessments, coordination, and implementation of age-appropriate social medicine services to meet the needs of patients, families, and significant others.

Key Responsibilities:
  • Participate as a member of the ED care team, providing social medicine expertise in response to care team referrals and case finding of high-need populations.
  • Complete psychosocial assessments of patients in the ED, identifying psychosocial needs, developing, and implementing a plan of care, and documenting according to departmental standards, Federal, and State regulations.
  • Assist patients and their support systems with coping strategies related to illness, providing information and referrals to community resources for post-discharge support.
  • Conduct social medicine consultations and assistance for cases involving abuse, neglect, advance care planning, and legal decision making.
  • Contribute to the patient discharge planning process, assessing patient and support system choices, educating them on the risks and benefits of their choices, and facilitating discharge.
  • Provide information, resource identification, and referral services, mediation services, advocacy services, and education to patients and their support systems.
  • Participate in performance improvement, implement Quality Improvement processes, and assist with the development and implementation of the department's annual work-plan.
Requirements:
  • Minimum one (1) year experience in a healthcare setting.
  • Masters degree in Social Work.
  • Licensed Clinical Social Worker (Hawaii) or Licensed Social Worker (Hawaii).
  • Basic Life Support certification from the American Heart Association.
