The Disease-Specific Program Coordinator plays a vital role in promoting optimal patient care for a disease-specific patient population at a site-based level. This position collaborates with physician and administrative leadership to integrate best practices throughout the organization.

Key Responsibilities:

  • Serve as a clinical expert resource for staff providing disease-specific patient care.
  • Facilitate team coordination in the provision of evidence-based care.
  • Develop and maintain disease-specific protocols and processes to support provision of care in conjunction with Service Line leadership.
  • Support patient and community education.
  • Prioritize work and effectively manage time and projects with minimal supervision.

Requirements:

  • Active RN Licensure in Tennessee.
  • Bachelor's Nursing degree.
  • CEN or CCRN certification preferred.
  • Related experience required.
