The Disease-Specific Program Coordinator plays a vital role in promoting optimal patient care for a disease-specific patient population at a site-based level. This position collaborates with physician and administrative leadership to integrate best practices throughout the organization.
Key Responsibilities:
- Serve as a clinical expert resource for staff providing disease-specific patient care.
- Facilitate team coordination in the provision of evidence-based care.
- Develop and maintain disease-specific protocols and processes to support provision of care in conjunction with Service Line leadership.
- Support patient and community education.
- Prioritize work and effectively manage time and projects with minimal supervision.
Requirements:
- Active RN Licensure in Tennessee.
- Bachelor's Nursing degree.
- CEN or CCRN certification preferred.
- Related experience required.
