Job Overview:


The Community Health Liaison will serve as an essential member of the Diversity and Inclusion Team at Community Medical Center. This role requires a strong understanding of cultural needs and a trusting relationship with the patient/family, enabling the liaison to facilitate effective communication and patient empowerment in interactions with healthcare systems.



Key Responsibilities:



  • Link patients to healthcare resources and help overcome barriers to obtaining necessary information and services
  • Facilitate communication and patient empowerment in interactions with healthcare systems
  • Document activities, services, and results in an effective manner, adhering to policies and procedures
  • Help the healthcare team become culturally relevant and responsive to the service population
  • Advocate for patient needs and provide informal counseling, support, and follow-up
  • Work collaboratively and effectively within a team, establishing positive relationships with patients/families and providing feedback
  • Effectively work with people from diverse backgrounds and build and maintain positive working relationships
  • Motivate patients to be active, engaged participants in their health
  • Continuously expand knowledge and understanding of resources, services, and programs provided, human relations, and procedures used in dealing with the public


Requirements:



  • High School or GED required
  • Strong understanding of cultural needs related to the community
  • Excellent problem-solving skills
  • Detail-oriented with attention to follow-up
  • Effective interpersonal, oral, and written communication skills
  • Working knowledge of MS Word and MS Outlook
  • Ability to work independently
  • Bachelor's Degree preferred
