We are seeking an experienced Case Management Specialist to join our team at Goodwill of Western & Northern Connecticut. As a Case Management Specialist, you will be responsible for coordinating and implementing case management services for individuals with developmental disabilities.

Job Overview

The successful candidate will have a strong understanding of case management principles and practices, with a minimum of 1 year of experience in direct care, case management or human services. Excellent communication and interpersonal skills are essential.

The estimated salary for this position is $50,000 - $60,000 per year, depending on experience. We offer a comprehensive benefits package, including health, dental, and vision insurance, as well as opportunities for career advancement and professional development.

Key Responsibilities
  • Coordinate and implement case management services for individuals with developmental disabilities
  • Assess participant needs and develop individualized service plans (ISPs)

Requirements
