Job OverviewThe successful candidate will have a strong understanding of case management principles and practices, with a minimum of 1 year of experience in direct care, case management or human services. Excellent communication and interpersonal skills are essential.

Key Responsibilities- Coordinate and implement case management services for individuals with developmental disabilities
- Assess participant needs and develop individualized service plans (ISPs)
