Job Title: Registered Nurse - Home Health

VNS Health is seeking a skilled Registered Nurse to join our Home Health team. As a member of our interdisciplinary care team, you will provide high-quality patient care and support to individuals in their own homes.

Job Summary:

The Registered Nurse will be responsible for assessing the health needs of patients, developing and implementing individualized care plans, and providing education and support to patients and their families. You will work collaboratively with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.

Responsibilities:
  • Conduct comprehensive assessments of patients' physical and emotional needs
  • Develop and implement individualized care plans to meet patients' unique needs
  • Provide education and support to patients and their families on health management and self-care strategies
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes
  • Document all patient interactions and care provided in accordance with VNS Health policies and procedures
Requirements:
  • Current registration to practice as a Registered Professional Nurse in New York State
  • Associate's Degree in nursing required; Bachelor's Degree in nursing preferred
  • Prior experience working with an aging population and providing clinical assessments of their health needs
