Job Summary:

We are looking for a dedicated and experienced Registered Nurse to work in a Skilled Nursing setting. The ideal candidate will have a strong background in clinical care and a passion for delivering exceptional patient outcomes.

Responsibilities:
  • Monitor and assess the delivery of home health field services to ensure high-quality patient care.
  • Supervise the activities of Home Health Aides (HHAs) and provide guidance and support as needed.
  • Collaborate with primary care physicians and clinical teams to develop and implement comprehensive care plans.
  • Conduct initial and ongoing supervisory field visits to patients' homes to provide assessment, training, and coordination of care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
  • Participate in quality assurance and education programs to stay up-to-date on best practices and industry developments.
Requirements:
  • License and current registration to practice as a Registered Professional Nurse in the State of New York.
  • CPR/BLS Certification.
  • Associate's Degree in Nursing from an approved program.
  • Minimum of two years in a clinical background as a Registered Nurse.
  • Home care experience in a licensed home care service agency or CHHA settings preferred.
