About the Role

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our adult and geriatric clients in their homes. As a member of our multidisciplinary healthcare team, you will work closely with nurses and other healthcare professionals to develop and implement individualized care plans that promote the health, safety, and independent living of our clients.

Responsibilities
  • Conduct home visits to assess clients' physical needs and develop personalized care plans.
  • Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
  • Implement evidence-based interventions to promote wound healing, improve mobility, and enhance overall physical function.
  • Collaborate with nurses and other healthcare professionals to revise and update care plans as needed.
  • Educate clients and their families on rehabilitative care and activities to promote independent living.
Requirements
  • A current Pennsylvania Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
