About the Job

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Chester, Pennsylvania. As a home health physical therapist, you will be responsible for providing high-quality rehabilitative care to adult and geriatric clients in their homes.

Responsibilities
  • Conduct home visits to assess clients' physical needs and develop personalized care plans.
  • Implement evidence-based interventions, such as manual therapeutic exercises, gait training, and balance assessments, to promote clients' independence and mobility.
  • Collaborate with nursing staff to develop and implement comprehensive care plans that address clients' physical, emotional, and social needs.
  • Educate clients and their families on rehabilitative care and activities necessary to promote healthy living.
  • Document observations, interventions, and evaluations in a state-of-the-art touch pad tablet.
Requirements
  • A current Pennsylvania Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
