Job Title: Physical Therapist for Home Health Care

About the Job:
BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Chester, Pennsylvania. As a Physical Therapist, you will be responsible for providing high-quality care to adult and geriatric clients in their homes. You will work independently, managing your time effectively and making home visits to clients in designated territories.

Responsibilities:
Perform diagnostic tests and measurements to assess clients' mobility, strength, and balance.
Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
Continuously assess and revise care plans, participating in multidisciplinary care planning as needed.
Educate clients and their families on rehabilitative care and activities to promote independent living.
Accurately document observations, interventions, and evaluations in state-of-the-art electronic medical records.

Qualifications:
Current Pennsylvania Physical Therapist license.
Minimum of one year of recent work experience as a Physical Therapist.
Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational, and problem-solving skills.
Solid computer skills, with prior experience with electronic medical records preferred.
