About the Role

We are seeking a skilled Hospice Nurse Practitioner to join our team at Gentiva. As an Advanced Practice Registered Nurse, you will play a critical role in providing high-quality patient care and supporting the hospice diagnosis and prognosis process.

Job Responsibilities
  • Perform Face-to-Face encounters with patients and their families to gather clinical information
  • Communicate findings to the hospice physician responsible for certifying or recertifying patient eligibility
  • Make recommendations to the Interdisciplinary Group (IDG) based on assessment findings

The ideal candidate will have a Master's degree in Nursing, completion of a graduate-level nurse practitioner education program, and current state licensure as a registered nurse and advanced practice nurse.

Requirements
  • Master's or higher degree in Nursing
  • Current state license as a registered nurse
  • Current national certification as a Nurse Practitioner (ANCC or AANP)
