Heartland Hospice, a leading provider of compassionate end-of-life care, is seeking a skilled Licensed Masters' Social Worker (MSW) to join our interdisciplinary team. The Role

This is a critical position within our organization, requiring a professional with expertise in psychosocial support, assessment, and intervention. The successful candidate will be responsible for providing empathetic and personalized care to patients and their loved ones, while working closely with other healthcare professionals to ensure comprehensive support.

Key Responsibilities
  • Conduct thorough assessments to identify patient and family needs, goals, and interventions.
  • Develop and implement individualized plans of care that address physical, emotional, and spiritual well-being.
  • Collaborate with the healthcare team to ensure seamless coordination of care and services.
  • Provide education and support to patients and families on disease management, coping strategies, and advance care planning.
Requirements
  • Masters' degree in social work from a CSWE-accredited institution.
  • Licensure as required by the state in which the hospice operates.
  • A minimum of one year of experience as a social worker in a healthcare or hospice setting, with three to five years preferred.
  • Demonstrated knowledge of terminally ill patients and their families, as well as understanding of hospice care and psychosocial dynamics of illness, loss, and death.
