Job Summary

We are seeking a highly skilled and compassionate Recreational Activities Coordinator to join our team at Pace of Southwest Michigan. As a key member of our interdisciplinary team, you will be responsible for planning and coordinating therapeutic and recreational activities for our participants.

Key Responsibilities
  • Conduct initial assessments and scheduled reassessments of all participants to understand their needs and preferences.
  • Develop and implement specific therapeutic programs that promote physical, mental, and social well-being across the cognitive and functional continuum.
  • Communicate participant changes and progress with the Interdisciplinary Team (IDT) members to ensure seamless care.
  • Plan and provide group and individual activities, including community outings and special events, to engage participants and foster social connections.
  • Record and document participant progress in the electronic medical record (EMR) and maintain accurate records.
  • Collaborate with placement facilities to assess participant activity and involvement, and work with facility staff to ensure continuity of care.
  • Create monthly and daily calendars to inform participants and staff of daily activities and events.
  • Participate as a member of the IDT and contribute to the development of care plans.
  • Coordinate with various community resources to provide participants with access to essential services.
  • Perform home visits as directed by the Day Center Supervisor or designee.
Requirements
  • High School Diploma or GED required.
  • Minimum of one (1) year experience in social or recreational programs providing and coordinating services for the frail and elderly population.
  • Valid MI Driver's License preferred; current CPR certification by AHA or willingness to obtain as offered by Pace of Southwest Michigan.
  • Ability to work a flexible schedule and travel in all kinds of weather.
  • Excellent attention to detail, organizational skills, and verbal and written communication skills.

The employee frequently is required to sit and use hands and fingers, handle or feel objects, tools or controls. The employee is occasionally required to stand and walk. The employee must be able to travel in all kinds of weather.

While performing the duties of this job, the employee is required to frequently move up to 50 pounds and perform physical demands expected while providing medical care for an elderly population. Specific vision abilities required by this job include close vision, distance vision and the ability to focus.
