Case Manager

We are seeking a highly skilled and compassionate Case Manager to join our team at Mid-Columbia Center for Living. As a Case Manager, you will play a vital role in providing case management services to assigned clients, assessing eligibility, and collaborating with agency professionals in case planning and service coordination.
Key Responsibilities- Provide case management services to assigned clients, including assessing eligibility and collaborating with agency professionals in case planning and service coordination.
- Assist clients in accessing services and resources needed to achieve treatment goals and maintain independence within the community.
- Coordinate with other community partners to develop, provide, and maintain a rich array of services and resources for clients.
- Participate in medication appointments, medication delivery, and accurate and timely documentation in client records.
- Provide transportation for clients when required.
- Actively participate in joint case planning and coordination with internal multi-disciplinary teams and agency psychiatrists.
- Meet minimum client service productivity requirements as set by the Agency to meet requirements of outside funding and/or reimbursement sources.
- Utilize agency clinical software systems to prepare current client records and reports as necessary and required.
- Any combination of education, licensure, and experience that demonstrates the ability to perform the position's essential responsibilities is qualifying.
- Two (02) years of responsible experience in human services is required, and a Bachelor's degree in a MHACBO qualifying field or equivalent training and work experience in human services or a related field is preferred.
- Experience working with the severe and persistent mentally ill population is preferred.
- Bilingual fluency in Spanish/English is preferred.
- Must possess a valid Oregon or Washington Driver's license and be insurable for client service purposes and for travel between business offices, meetings, and/or trainings.
- Competitive salary, dependent on experience.
