We are seeking a highly skilled and compassionate Case Manager to join our team at Mid-Columbia Center for Living. As a Case Manager, you will provide critical support to clients with mental illness and addictions, helping them access necessary services and resources to achieve their treatment goals and maintain independence in the community.
Key Responsibilities- Assess eligibility of potential clients and assist them in acquiring services and resources needed to achieve treatment goals and maintain independence in the community.
- Provide case management services to assigned clients, including transportation and medication delivery as needed.
- Collaborate with internal multi-disciplinary teams and agency psychiatrists to develop and implement effective case plans.
- Meet minimum client service productivity requirements and maintain accurate and timely documentation in client records.
- Participate in team and agency meetings, as well as local, regional, and state meetings as requested.
- Two years of responsible experience in human services, with a Bachelor's degree in a MHACBO qualifying field or equivalent training and work experience.
- Experience working with the severe and persistent mentally ill population is preferred.
- Bilingual fluency in Spanish/English is preferred.
- Valid Oregon or Washington Driver's license and insurability for client service purposes and travel between business offices, meetings, and trainings.
- Pass all criminal history check requirements as required by ORS 181.536-181.537 and in accordance with OAR 407-007-0200 to 407-007-0370.
- Competitive salary, dependent on experience.
