Job Overview – Case Manager
Join our client's team as a Case Manager in Bronx, NY, where you will play a crucial role in delivering impactful case management services. In this role, you'll conduct comprehensive assessments, develop tailored service plans, and engage in fieldwork to support clients in achieving their goals and maintaining independent living.

Location: Bronx, NY
Compensation: $25/hour
Schedule: 9:00 AM to 5:00 PM, Monday to Friday


Responsibilities as the Case Manager:
  • Client Support: Deliver tailored case management services, ensuring clients receive personalized support that meets their needs.
  • Assessment & Planning: Conduct comprehensive assessments and develop service plans with achievable goals, adjusting as necessary.
  • Client Interaction: Perform regular home and office visits to monitor client progress and provide support.
  • Health & Wellness: Assist clients in accessing healthcare and promoting daily living activities.
  • Crisis Management: Provide effective crisis intervention to address immediate client needs.
  • Housing Assistance: Offer guidance on tenant rights, support housing transitions, and ensure financial compliance.
Qualifications for the Case Manager:
  • Education: Bachelor’s Degree in Social Work or Public Administration preferred (relevant experience considered).
  • Experience: 2+ years of experience in counseling, particularly with mental health and substance abuse populations.
  • Skills: Strong organizational and communication skills with a compassionate approach.
  • Technical Proficiency: Basic computer skills, including Microsoft Excel and Word.
  • Language: Bilingual in Spanish is a plus
